What is the Support Report Respect disclosure form?

If you are a student or colleague of Leeds Beckett University, you can use this form to disclose an incident anonymously. You can also use this form to disclose on behalf of a student or colleague of Leeds Beckett University. 

You can use this current form to disclose behaviours or incidents of bullying, harassment, sexual misconduct (including sexual violence), domestic abuse/relationship issues or any other form of unacceptable behaviour. Completing this form lets the university know that something has happened. However, this will not instigate a formal investigation or complaint. 

What happens after my disclosure?

The information in this submission will be received by a specially trained Student Harassment and Sexual Misconduct Adviser in Student Services, or Dignity and Respect Adviser in Equality, Diversity & Inclusion (if you are an LBU colleague disclosing on behalf of yourself or another colleague). 

As you are choosing to disclose an incident anonymously, no contact details are recorded and you will receive no follow up on submission of your disclosure. 

Important: this form should not be used to access urgent support.

  • If you are on campus, you should contact emergency services on 999 first, then inform Security Services on their 24-hour emergency number 0113 812 3165.
  • If you are not on campus, you should contact emergency services by calling 999.

How does Leeds Beckett use my data?

Leeds Beckett University ( LBU ) are the Data Controller for the information you provide when completing this form. Your details will be held in accordance with UK Data Protection legislation for the purpose of addressing your Support Report Respect disclosure form. While completing the form, we will not ask for any identifying personal information. However, we will ask for you to inform us of your link to the university, and if you are disclosing on behalf of someone else, what your relationship is to the victim/survivor. This information is solely used to identify any trends in disclosures, and better understand the prevalence of unacceptable behaviour amongst our university community.
