Colleague Reporting

This form is for employees or for an individual who wishes to seek advice or make a report on behalf of a colleague. The student version of this form (either for students or for those making a disclosure on behalf of a student) can be found here.

You can use this form to report bullying, any form of harassment or sexual misconduct (which also includes sexual violence) and to contact an advisor.

The information in this report will go to an adviser in the EDI Team. Our advisers will provide you with initial support following an incident, provide you with advice and explain options if you would like to make a formal complaint. We aim to respond to you within 3 working days.

Completing this reporting form lets the university know that something has happened. However, this will not automatically instigate an investigation or formal procedure and instead advisors will offer support and talk to you about your options.

The information you provide will only be seen by the EDI Team unless there is a need to pass it on to other relevant people who are in a position to help resolve issues, but this will be with your permission. This will be done on a strictly need to know basis and with the highest degree of confidentially.

In certain, limited circumstances, the University may judge that action is required even if you are not requesting any. This will be done in accordance with our duty of care guidance , for example if there is reason to believe that an individual is at significant risk of harm. In addition to being able to speak with an Dignity & Respect Advisor, the information you share in this form allows the university to undertake anonymised statistical monitoring, which helps us monitor trends and undertake prevention and intervention work..
