Student Name Change Request Form 2026/27

What is this form for?

You can use this form to request an update to your name used in university systems and communications. We understand that students may want to change their name for a variety of reasons. In this form, you will be able to request either a preferred or a legal name change, an update to title, pronouns, gender, student email address, and campus card. 

If you have any difficulties in completing this form, you can visit a Student Services Centre on either campus or call us on 0113 812 3000 to speak with an Officer, who can complete it with you.

How does Leeds Beckett use my data?

Leeds Beckett University ( LBU ) are the Data Controller for the information you provide when completing this form. Our university is committed to maintaining the confidentiality of our students, and all conversations regarding personal detail updates are on a need-to-know basis.  For more information please visit the university’s Privacy Statement. Please note, whilst the Privacy Statement does not cover the collection of information on marital status, this information would be used for local data purposes and to align with submitted evidence for legal name changes.

If you only have 1 name, please enter this in the 'last name' box and type 'N/A' in 'first name' and 'middle name'. We are required to record data in the first name box, and the format we suggest allows the Registration and Awards team to identify students with one name and ensure certificates are printed correctly.

Legal Name Change Evidence

If you are changing your name legally, please attach your evidence, this can be a scan or photo of the document. We accept deed polls (enrolled or unenrolled), passports, marriage certificates, civil partnership certificates, or decree absolutes. If you are reverting to a maiden name, please ensure you supply us with documentation that shows your maiden name such as a birth certificate.

Please note, any deed poll issued from 2024 onwards must have 2 witnesses to comply with government guidance.

Student Email Address Update

We are able to update your student email address if your initials/last name have changed. 

Please note, any links to documents etc. that have been shared with your student email address prior to the change, will no longer work. You should request access to these links and provide your updated email address to the person that has shared the link if you encounter any difficulties.

Would you like your campus card updated?

We are able to update your name on your campus card to reflect your name change. We will confirm via email when you are able to pick your new campus card up, from either the Student Services Centre at City Campus (opposite the Leslie Silver Library), or the Student Services Centre at Headingley Campus (Priestly Building ground floor).

Do you have a Leeds Beckett Gym or Athletic Union membership?

If you have a gym or athletic union membership with Leeds Beckett and have requested a name change, please let us know as this could impact your access.
