We're looking to make improvements to our sport and physical activity offers, activities and services and would love to get your thoughts to help shape our future.
We've created an online survey for you to take part in, and as a thank you for taking part you will be entered into a prize draw for a £50 Amazon voucher.
The survey is quick and simple for you to complete, with 20 short answer questions. Your answers will be sent to a link at SurveyMonkey.com where data will be stored in a password protected electronic format. Survey Monkey does not collect identifying information such as your name, email address, or IP address.
Your survey responses will remain anonymous. No one will be able to identify you or your answers. The £50 Amazon prize draw will be separate to your survey answers, and you must provide your email address for the purpose of the competition.
